Three options:

1. Fill out application and mail to:

2. Fill out application and leave (in an envelope) for the driver to pick up at your next delivery.
3. Call 518-854-9400 and place your information with us. Then you would ONLY sign the application (Do NOT put any other information on it) and either mail to the above address or leave (in an envelope) for the driver to pick up at your next delivery.
